Patient Information Pathways
The Mid Trent Cancer Network is in the process of producing electronic patient information pathways and for these to be populated with the leaflets and booklets and other resources which have been agreed by the Network Site Specific Group as a Core Patient Information Pathway.
The Mid Trent Cancer Network wants these pathways to:
* List exactly what information is being offered by hospital Trusts and other organisations producing patient information at key stages in the patient pathway and include contact details and links for obtaining that information
* Be made widely available to all healthcare professionals involved at every stage in the patient pathway. For the first time, health professionals have easy access to a ready menu of information resources to offer their patients and carers in a variety of treatment and care settings that ensures patient information is part of the total package of care they provide
* Provide patients and carers with a reliable and comprehensive range of up to date information materials. In time, the electronic patient information pathways will also be available from a range of local and national public websites including NHS Direct Online and NHS UK so that when patients and carers are online, they can view the information resources available from their local cancer network.
Patient Information Pathways in place
The Mid Trent Cancer Network has three Cancer Patient Information Pathways developed and agreed:
- Head and Neck Cancer Patient Information Pathway
- Gynaecology Cancer Patient Information Pathway
- Lung Cancer Patient Information Pathway
Patient Information Pathways in development
The Mid Trent Cancer Network is currently developing the following Information Pathways:
- Colorectal Cancer Patient Information Pathway
- An Information Pathway for Carers
- End of Life Care Information Pathway (including palliative care).
Further pathways are planned for development in 2008.
